Almost everyone says handwashing matters. Far fewer people do it well, and almost no one measures it the way you would expect. We pulled together the best public data on how Americans wash their hands, how it affects kids and school, and one surprising gap: there is no such thing as a state-by-state handwashing ranking, because the survey that would produce one does not ask the question.
The finding
The United States has no recurring, state-representative measurement of handwashing behavior. The federal survey that produces state-by-state health rankings does not ask about handwashing at all, so every national handwashing figure you have seen is either a one-off study, a private survey, or a food-safety question. We document exactly which surveys do and do not measure it below.

The handwashing honesty gap
The most revealing thing in the data is not any single number. It is the distance between what people say about handwashing and what researchers see when they quietly watch. Almost everyone endorses it. Far fewer do it well. Here is that gap, side by side.

Why the two sides never match. People are not simply lying. Surveys capture what people believe they should do, and most people genuinely intend to wash. Observation captures what actually happens when no one is asking, and behavior slips when people feel unwatched. So the useful question about any handwashing statistic is whether it came from a survey or from someone counting.
Why it matters: kids, illness, and school
A note on what follows. The figures in this section are context, not handwashing measurements. They describe how much illness costs kids and schools, which is why handwashing behavior is worth measuring in the first place. They are not evidence about how much handwashing itself changes these outcomes. That evidence comes later, in "Does handwashing actually work."
For families, the reason any of this matters is simple: sick kids miss school, and missed school adds up. Illness is the leading single reason children are absent. In the most detailed state dataset available, from Rhode Island, roughly three quarters of all absences were tied to illness, and that share rose slightly after the pandemic (Fordham Institute).
The illnesses that spread through classrooms and daycares are exactly the ones hands carry. Norovirus, the classic stomach bug, caused about 64% of school outbreaks in a large CDC review of school and childcare settings (CDC, 2009 to 2020 data). Flu drove a brutal 2024 to 2025 season, with 279 pediatric deaths, the most ever recorded outside a pandemic year (CDC). And hand, foot and mouth disease, which mostly hits young children, has surged: medically seen cases roughly tripled between 2023 and 2025 (Epic Research).
Chronic absenteeism by state, 2022 to 2023
Since illness is the top cause of missed school, and since no state tracks handwashing directly, chronic absenteeism is the closest thing to a state-level view of the problem. This is the cleanest all 50 state dataset that exists, from the US Department of Education. It counts every student who missed 10% or more of the school year, for any reason, so read it as a picture of lost school time, not a handwashing scoreboard. Sort it, or search for your state.

Does handwashing actually work?
Yes, and the honest version is more useful than the marketing version. Decades of school studies show real reductions in illness-related absences when kids wash or sanitize regularly, but the effect is uneven. It is strong and consistent for stomach illness, and smaller and less certain for coughs and colds.
A widely cited meta-analysis found handwashing cut stomach illness by about 31% and respiratory illness by about 21% (Aiello et al., 2008). The landmark global review from Cochrane, pooling more than 61,000 people, put the reduction in respiratory infections at about 16% (Jefferson et al., 2020). A strong independent US classroom trial found a clear drop in stomach-illness absences but no significant change in respiratory absences (Sandora et al., Pediatrics, 2008).
What handwashing does, and does not do

A caveat worth knowing. Many of the eye-popping early numbers, the ones showing 40% or 50% reductions, came from studies funded by the companies selling the sanitizer. Independent trials tend to show smaller effects, and one rigorous New Zealand trial found no added benefit from sanitizer at all (Priest et al., 2014). Handwashing helps. It is not magic, and the strongest, cleanest benefit is against stomach illness.
The data America does not collect
Here is the finding that surprised us most. You can rank all 50 states on obesity, smoking, exercise, and dozens of other health behaviors, because the CDC runs a giant state-representative survey called BRFSS that asks about them. It does not ask a single question about handwashing (2022 BRFSS questionnaire).
Because of that one gap, there is no recurring, state-representative handwashing dataset anywhere in the United States. Every "how often do Americans wash" number you have seen is national, from a private survey, or from a one-off study. We went through the major national surveys to document exactly which measure handwashing behavior and which do not. As far as we can find, this audit does not exist anywhere else.

The other honest limits worth knowing:
- The "three quarters of absences are illness" figure comes from Rhode Island only, not a national count.
- CDC's rapid norovirus network, NoroSTAT, includes 14 participating states, and its flu hospitalization network, FluSurv-NET, covers about 14 sites. CDC's outpatient flu tracking, ILINet, is national. So "14 states" applies to specific surveillance networks, not to flu surveillance as a whole (NoroSTAT).
- Hand, foot and mouth disease is not nationally tracked, so the best outbreak data is single-state.
- No US study has ever measured how much hand, foot and mouth disease drives school absence. It is a genuine gap.

Methodology
We set out to gather the most reliable public data on US handwashing behavior and its links to child illness and school. Sources were prioritized in this order: federal agencies (CDC, US Department of Education), peer-reviewed studies and systematic reviews, and named national surveys with disclosed sample sizes and methods. We excluded brand marketing pages, undated claims, and figures we could not trace to an original source. Where a widely quoted statistic came from a single state or a single venue, we say so rather than presenting it as national. Where sources disagreed, we noted the discrepancy. This is a curation-and-synthesis piece: the underlying data belongs to the agencies and researchers cited, and the one original contribution is the audit of which national surveys do and do not measure handwashing.
